Cidofovir is used for the management of CMV retinitis in patients with acquired immunodeficiency syndrome (AIDS).
Recommended Dosage:
Induction Therapy: The recommended dose is 5 mg/kg body weight (administered as an intravenous infusion at a constant rate over 60 minutes) administered once weekly for 2-consecutive weeks.
Maintenance Therapy: Beginning 2-weeks after the completion of induction therapy, the recommended maintenance dose is 5 mg/kg body weight (given as an intravenous infusion at a constant rate over 60 minutes) administered once every 2-weeks.
Suspension of maintenance therapy with cidofovir should be considered as per the with local recommendations for the management of HIV infected patients.
- Cases of acute renal failure resulting in dialysis or contributing to death have occurred with as few as one or two doses of Cidofovir. Serum creatinine and urine protein must be monitored within 48 hours before each dose of Cidofovir.
- Neutropenia may occur during treatment with cidofovir 375 mg injection. Neutrophil count needs to be monitored while receiving cidofovir treatment.
- Decreased intraocular pressure may emerge while on Cidofovir therapy, and in some instances has been related to reduced visual acuity. Monitor intraocular pressure during therapy.
- Reduced serum bicarbonate related to proximal tubule injury and renal wasting syndrome (including Fanconi’s syndrome) have been seen in patients receiving cidofovir.
- Because of the potential for the increased nephrotoxicity, doses of cidofovir greater than the recommended should not be administered and the frequency or rate of administration should not be exceeded.
- Cidofovir injection is formulated for intravenous infusion only and should not be administered by intraocular injection. Administration of this drug by infusion should be accompanied by oral probenecid and intravenous saline prehydration.
- Uveitis or iritis was observed in clinical trials and during postmarketing in patients with Cidofovir therapy. Treatment with topical corticosteroids with or without topical cycloplegic agents need to be considered. Monitor patients for signs/symptoms of uveitis/iritis during Cidofovir therapy.
- Patients receiving vistide injection should have regular follow-up ophthalmologic examinations. Patients may also experience certain other manifestations of CMV disease despite therapy with vistide.
- Women of childbearing age/potential should be advised that cidofovir is embryotoxic in animals and should not be used during pregnancy. Women of childbearing potential should use apt contraception during and for 30 days following treatment with Cidofovir.
