Syprine (Trientine)
- Medicine Name: Syprine
- API: Trientine Hydrochloride
- Dosage Form & Strength: Capsules: 250 mg
- Manufactured By: Valeant Pharma, Inc.
Medical Uses: Syprine (trientine hydrochloride) is primarily used for the treatment of Wilson’s disease (WD), particularly in patients who are intolerant to penicillamine. Wilson’s disease is a genetic disorder that causes excessive accumulation of copper in the liver, brain, and other vital organs. Syprine helps to remove excess copper from the body, thereby preventing copper toxicity and its associated complications.
Recommended Dosage: The recommended initial dose is 500 to 750 mg/day for pediatric patients and 750 to 1250 mg/day for adults given in divided doses 2, 3, or 4 times daily. This may be increased to a maximum of 2000 mg/day for adults or 1500 mg/day for pediatric patients aged 12 or under. The daily dose should be increased only when the clinical response is not adequate or the concentration of free serum copper is persistently above 20 mcg/dL. Optimal long-term maintenance dosage should be determined at 6-12 month intervals.
Syprine must be given on an empty stomach, at least 1 hour before meals or 2 hours after meals, and at least one hour apart from any other drug, food, or milk. The capsules should be swallowed whole with water and should not be opened or chewed.
- Patients may experience hypersensitivity reactions including fever, rash, and eosinophilia. Stop the use immediately if hypersensitivity reactions occur.
- Syprine 250 mg capsules can cause iron deficiency. Monitor iron levels regularly, especially in patients with a history of anemia.
- Some patients may experience neurological worsening during treatment. Monitor neurological status closely.
- Regularly monitor liver function tests, serum copper, ceruloplasmin, and urinary copper excretion to assess the effectiveness of the treatment.
- Do not take iron supplements within two hours of taking Syprine, as they can interfere with its absorption.
- Use caution when prescribing Syprine to pregnant or breastfeeding women. Weigh the potential benefits against potential risks.
- Use caution in patients with renal impairment as the medicine is excreted via the kidneys. Adjust dosage if necessary.
- Trientine can cause gastrointestinal side effects such as nausea and vomiting. Take the medicine with food if stomach upset occurs, unless otherwise directed by a doctor.
