Ocaliva is a farnesoid X receptor (FXR) agonist used in the treatment of adult patients with primary biliary cholangitis (PBC):

• without cirrhosis or

• Start with an Ocaliva dosage of 5 mg once a day for the initial three months.
• Following the initial 3 months, for individuals who have not reached an appropriate reduction in ALP or total bilirubin and those who are tolerating the medicine Ocaliva, enhance to a maximum dosage of 10 mg once a day.

Take Ocaliva either with or without meal/food. For patients with bile acid binding resin, take Ocaliva 5mg or 10mg at least 4 hours prior to or 4 hours following the use of bile acid binding resin, or at as great an interval as possible.

• Hepatic failure and decompensation, fatal or resulting in liver transplant may occur with Ocaliva treatment in PBC patients with cirrhosis.
• Routinely assess patients for progression of primary biliary cholangitis, including hepatic side effects, with laboratory and clinical evaluations to determine whether drug discontinuation is required.
• Severe pruritus may occur with Ocaliva 5 mg, 10 mg tablets. Consider clinical assessment of those with new onset or worsening severe pruritus. 
• Patients with primary biliary cholangitis generally exhibit hyperlipidemia characterized by a significant rise in total cholesterol primarily because of elevated levels of HDL-C (high-density lipoprotein-cholesterol).
