Ninlaro is formulated as a proteasome inhibitor (PI) used in conjunction with medications lenalidomide and dexamethasone to treat individuals with multiple myeloma (MM) who have previously been treated with at least one therapy.
Recommended Dosage: The recommended beginning dosage of Ninlaro capsule is 4 mg administered orally once weekly on Days 1st, 8th, & 15th of a 28-day treatment cycle. The recommended starting dose of the drug lenalidomide is 25 mg administered daily on Days 1st through 21st of a 28-day treatment cycle. The recommended starting dose of dexamethasone is 40 mg administered on Days 1st, 8th, 15th, and 22nd of a 28-day treatment cycle.
Ninlaro needs to be taken at least 60 minutes before or at least 120 minutes after food. Capsules should be swallowed whole with water. Crushing, chewing, or opening the capsule is not recommended. In case of puking after taking a dose of Ninlaro, do not repeat the dose.
- Monitor platelet counts monthly throughout treatment with Ninlaro capsules. Perform more frequent monitoring during the initial 3 cycles. Thrombocytopenia may occur and need to be managed with the help of dose modifications as well as platelet transfusions.
- Cases, sometimes maybe fatal, of thrombotic microangiopathy, including hemolytic uremic syndrome/thrombotic thrombocytopenic purpura (HUS/TTP) may occur in all those who are on treatment with Ninlaro. Assess for signs and symptoms of HUS/TTP. If the diagnosis is confirmed, stop treatment and evaluate.
- Drug-induced hepatocellular injury, liver injury, hepatic steatosis, hepatitis cholestatic, and hepatotoxicity may occur in those treated with Ixazomib. Hepatic enzymes must be monitored regularly and adjust Ixazomib dosing for Grade 3/4 symptoms.
- Ninlaro 4 mg, 3 mg, or 2.3 mg capsules can cause fetal harm if administered during pregnancy on behalf of the mechanism of action and outcome in animal studies. Males with female partners of reproductive age should use relevant contraception during treatment and for 90 days after the final dose.
