Symdeko is a fixed dose combination (FDC) of tezacaftor and ivacaftor, used to treat patients with cystic fibrosis (CF) aged 12 years and older who have a couple of copies of the F508del mutation, or who have at least one mutation in the cystic fibrosis transmembrane conductance regulator (CFTR) gene that is responsive to Symdeko (tezacaftor/ivacaftor).
If the patient’s genotype is not known, a CF mutation test should be considered in order to detect the existence of a CFTR mutation followed by verification with bi-directional sequencing when required by the mutation test instructions for use.
Recommended Dosage: The recommended dosage for Adults, Adolescents, and Children Aged 12 Years and Older is one tablet (tezacaftor 100 mg/ivacaftor 150 mg) taken in the morning and one tablet (ivacaftor 150 mg) taken in the evening, around 12 hours apart. Medicine Symdeko is for oral use. Patients should completely swallow the tablets. Take with fat-containing food, such as foods or snacks that contain fat and those prepared with oils or butter or those containing cheeses, nuts, eggs, whole milk, or meats.
In case 6 hours or less have passed since the missed or skipped morning or evening dose, the patient should take the missed/skipped dose as promptly as possible and continue on the standard schedule.
In case more than 6 hours have passed since the missed or skipped morning or evening dose, the patient should not take the missed/skipped dose. Take the very next scheduled dose of Symdeko at around the usual time. It is not advisable to take more than a single dose at the same time.
- Elevated transaminases may be observed in those with CF treated with symdeko tablets, as well as with ivacaftor monotherapy. Assessments of ALT and AST are required for all patients before the start of treatment, every 90 days during the first year of treatment, and annually thereafter. For those with a history of transaminase elevations more frequent monitoring is required.
- Exposure to ivacaftor is significantly reduced and exposure to tezacaftor may be decreased by the concomitant administration of CYP3A inducers, which may scale down the therapeutic effectiveness of medicine symdeko. Therefore, do not co-administer this medication with strong CYP3A inducers.
- Cases of non-congenital lens opacities may occur in pediatric patients treated with drug symdeko, as well as with ivacaftor monotherapy. A possible risk attributable to therapy with Tezacaftor/Ivacaftor and Ivacaftor cannot be excluded. Perform baseline and follow-up ophthalmological examinations in pediatric patients starting therapy with Tezacaftor/Ivacaftor and Ivacaftor.
- Do not take (tezacaftor 100 mg/ivacaftor 150 mg) if you take certain medications or herbal supplements such as: antibiotics such as rifabutin (Mycobutin), rifampin (Rifater, Rifamate), seizure medicines such as carbamazepine, phenobarbital (Carbatrol, Tegretol, Equetro), or phenytoin (Phenytek, Dilantin), snd St. John’s wort.
- Symdeko treatment may cause dizziness in some individuals who take it. Avoid driving a car, using machinery, or doing anything that requires full alertness until they know how this medication affects them. Avoid food or drink that contains grapefruit or Seville oranges while on Symdeko treatment.
- The medicine Symdeko should always be taken with a fat-containing food/meal or snack to help the body absorb the medication. Be sure to avoid meals/foods and drinks that are known to contain grapefruit while taking Symdeko because they may be responsible for affecting the existence of Symdeko in the body.
